Title:Community Support Professional

Type of Employment: Per Diem

Pay Rate: $16.19l

POSITION CONCEPT:

The Community Support Professional will provide person-centered, face-to-face community habilitation and/or respite services, which will be scheduled based on the individual’s needs. Services can take place in the individual’s home and/or the community.

POSITION RESPONSIBILITIES:

The Community Support Professional will:

  1. Actively cooperate with the President/CEO and other staff in a joint endeavor in satisfactorily achieving the goals of CWI.
  2. Work under the supervision of the Community Support Coordinator to implement Community Habilitation/ Respite services as specified in the Life Plan (LP) and the Staff Action Plan (SAP).
  3. Carry out the methodologies and techniques of each service area as written in the Staff Action Plan.
  4. Supervise, support and provide training to individuals that will encourage increased independence, individualization, community integration and productivity in concert with their self-identified valued outcomes and goals.
  5. Assist individuals displaying deficits in the areas of self-help, socialization, and adaptive skills.
  6. Interact with individuals, host families, and/or roommates in a therapeutic manner to facilitate social and interpersonal development.
  7. Keep record of individuals’ progress as instructed and within designated timeframes, including but not limited to, Electronic Visit Verification, service linked notes and general chart notes in AWARDS, electronic health record system.
  8. Attend and provide input during individuals’ reviews and individual’s Life Plan development as requested.
  9. Take part in any in-service training as required by the agency, and/or deemed necessary by the Manager.
  10. Perform other duties and responsibilities as assigned by the Community Services Manager and/or Community Support Coordinator.
  11. Act in any other capacity as directed by the President/CEO.

POSITION QUALIFICATIONS:

Community Support Professionals must have a high school diploma, or equivalent, good character and background and an acceptable driver’s license.  Demonstrated competency in First Aid/CPR, safety procedures and computer literacy a plus.  Must be highly responsible, able to work independently, with at least one year of experience working with disabled population.  Must have an acceptable driver’s license.
