Schedule: Monday-Friday 8:00am-4:30pm, flexibility required based on programmatic need

Salary: $30.90 – $32.13 per hour

Job description

The Health Services Specialist/ RN is responsible for overseeing the healthcare of program participants and implementing health services within the organization.

The Health Services Specialist/ RN Position Responsibilities:

1. Actively cooperate with the President/CEO and other staff members to achieve the goals of Community, Work, and Independence.

2. Implement a participant health care education program, including:

· Reviewing and maintaining participant medical records.

· Performing participant intake assessments in the area of health.

3. Act as a liaison with family physicians, dentists, and other health consultants, as well as with other community health agencies.

4. Under the discretion of the Department Head, the Health Services Specialist/ RN will be assigned to specific participant service areas(s), taking responsibility for health needs assessments, and development of service plans, implementation of service plans, and documentation and records maintenance.

5. Submit necessary reports related to health services.

6. Perform other duties and responsibilities as assigned by the Department Head.

7. Participate in any in-service training offered by the agency and attend meetings as requested.

8. Act in any other capacity as directed by the President/CEO.

Health Services Specialist/ RN Position Qualifications:

· Registered Nurse with New York State License.

· One year of experience preferred.

· Additional experience with the developmentally disabled is desired.
